How to Register to Retrieve Patient Controlled Substance Usage Reports

To register for Report Retrieval (pulling patient reports), please complete the Notarization Form in the Report Retrieval Registration Kit and send to AK PDMP to begin the registration process.

Alaska Announcement Letter June 2011
Informs pharmacist that RelayHealth was awarded an exclusive contract to enhance and begin managing data collection for the Alaska Prescription Drug Monitoring Program. The AK PDMP is now available for pharmacies to register and begin submitting data. Pharmacies should complete the registration process on the new system and begin submitting all controlled substance reporting data to that new system by August 1, 2011.

Request for Paper Submission of Data to AK PDMP

    Allows for a waiver from the electronic submission of data based on these parameters:
  • suffering a hardship created by a natural disaster or other emergency beyond the control of the dispenser and prevents the dispenser from satisfying 12 AAC 52.865(b).
  • dispensing in a controlled research project approved by an accredited institution of higher education or under the supervision of a governmental agency. Please attach a description of the research project.
  • Dispenses less than 10 prescriptions of controlled substances a month.
  • is located in an area where there is no access to the telecommunication services needed to comply with 12 AAC 52.865(b).
  • will suffer financial hardship if required to acquire the technology necessary to comply with 12 AAC 52.865(b).

Certification of No Controlled Substances Dispensed

    Certification that the pharmacy does not dispense controlled substances. Submittal of the forms certifies the following information:
  • My pharmacy, listed above, does not currently deliver any schedule II, III, IV, or V controlled substances to ultimate users who have an Alaska State address within the last twelve (12) months.
    • From _____________20___ to _____________20___
  • If my pharmacy's business practice changes regarding the dispensing controlled substances to ultimate users with an Alaska State address, we will notify the AK PDMP and begin data submission as required in AS 17.30.200.
  • My pharmacy will resubmit this form to the AK PDMP every calendar year, no later than February 1, in order to recertify that my pharmacy does not deliver any schedule II, III, IV, or V controlled substances to ultimate users who have an Alaska State address.
