How to Register to Retrieve Patient Controlled Substance Usage Reports
To register for Report Retrieval (i.e., pulling patient reports), please complete the Notarization Form in the Report Retrieval Registration Kit and send to MS PMP to begin the registration process.

Request for a Waiver or an Exemption From Reporting
If a dispenser believes they are exempt from data submission they should complete the Request for a Waiver or an Exemption from Reporting form and fax it to MS PMP at
